Breast hypertrophy is characterized by an increase in tissue volume without underlying pathology and often requires reduction mammaplasty. Surgical safety depends on preserving the blood supply to the nipple-areola complex (NAC). In this study, 50 patients with breast hypertrophy underwent preoperative duplex ultrasound to assess NAC vascularization. Significant variability and asymmetry in vascular architecture were observed, with the medial vascular pedicle identified as the most stable source. The use of duplex ultrasound reduced NAC necrosis risk to 1%, enabled individualized selection of the dermoglandular pedicle, and improved the safety of reduction mammaplasty.
