IMPROVEMENT OF METHODS OF CONSERVATIVE TREATMENT OF SPLEEN INJURIES IN CLOSED ABDOMINAL TRAUMA

Abstract

To improve the comprehensive approach to managing splenic injuries in cases of closed abdominal trauma.

      Materials and Methods

Between 2014 and 2024, 29 patients with splenic injuries resulting from multiple closed abdominal traumas were treated at the Samarkand branch of the Russian National Center for Emergency Medical Aid (RNEMA). All patients received conservative hemostatic therapy.

      Upon hospital admission, all patients with splenic injuries exhibited clear clinical signs of intra-abdominal bleeding, including abdominal pain, collapse, and positive signs such as Kerr's sign, the "roly-poly" symptom, and Kulenkampf’s sign. Ultrasound examination revealed the following volumes of hemoperitoneum:

In the latter two cases, blood was successfully removed via video laparoscopy.

     Conclusion

      Based on dynamic monitoring data from ultrasound, CT, and MRI, it was possible to conservatively manage splenic trauma in all 29 patients, achieving favorable clinical outcomes without mortality. In these 29 cases of closed liver and splenic injuries, modern non-invasive diagnostic methods allowed for accurate diagnosis and continuous monitoring. These methods proved sufficient in most cases, eliminating the need for video laparoscopy or surgical intervention.
