ORGAN-PRESERVING TACTICS IN CASE OF OVARIAN TORSION IN ADOLESCENT GIRLS

Abstract

Ovarian torsion is an urgent condition and has been an indication for ovarian removal for many years, but in recent years there have been studies on organ-preserving tactics in adolescents. Objective. to substantiate proposals for organ-preserving treatment for ovarian torsion in adolescent girls. Materials and methods. The study included 110 girls aged 10 to 19 years who were admitted and operated on for ovarian torsion. Results. Torsion of ovarian formations was more often observed in late adolescence (from 15 to 19 years) – 68 girls (61.8%). In 69 (62.7%) cases, torsion was found in the right ovary.  Torsion was 92.2% more likely to occur when the size of the tumor or tumor-like formation of the ovary was more than 50 mm. The sensitivity of ultrasound to determine ovarian torsion was 62%, the specificity was 88%. In laparoscopic surgery, organ-preserving tactics were used in 88.6% of cases, and in laparotomy surgery, organ-preserving tactics were used in 61.2% of cases. In adolescents with ovarian torsion, tumor–like formations - functional cysts - were most common - 63 (57.3%), germinogenic benign tumors were in second place - 20 (18.2%). Conclusions: In the surgical treatment of torsion of tumorous and benign ovarian tumors in adolescents, detorsion and organ-preserving surgery are effective in most cases.
