COMPARISON OF THE CLINICAL AND RADIOLOGICAL EFFICACY OF EPOXY SILERS OF TRADITIONAL COMPOSITION AND WITH ADDITIVES OF QUATERNARY AMMONIUM

Abstract

The success of endodontic treatment largely depends on thorough sanitation and hermetic obturation of the root canal system; however, the persistence of Enterococcus faecalis often leads to chronic apical periodontitis and relapses. Epoxy sealers provide favorable physico-chemical properties, but their antimicrobial activity is limited; the incorporation of quaternary ammonium compounds enhances the antibacterial potential of the material, which determined the relevance of this study. Objective: to compare the clinical and radiological efficacy of traditional epoxy sealers and sealers with quaternary ammonium additives in the treatment of chronic apical periodontitis. Materials and methods: the study included 20 patients divided into 2 groups (control, n=10 — traditional sealer; main, n=10 — modified sealer). Standard endodontic treatment with lateral condensation obturation was performed; clinical evaluation was carried out at 1, 3, 6, and 12 months, and radiological evaluation at 12 months; statistical analysis was performed using the χ² test (p<0.05). Results: symptoms in the main group occurred less frequently and resolved faster; after 12 months, complete healing was observed in 80% of patients compared to 50% in the control group (χ²=4.00; p=0.045). Conclusions: the use of epoxy sealer with quaternary ammonium compounds provides superior clinical and radiological outcomes compared to the traditional sealer, which is associated with its pronounced antimicrobial activity; however, further studies with larger samples and longer follow-up are required.
