DEVELOPMENT OF A DIAGNOSTIC AND TREATMENT ALGORITHM FOR DIFFERENTIATED THYROID CANCER BASED ON THE APPLICATION OF MOLECULAR-GENETIC ANALYSIS METHODS

The study evaluated the frequency and clinicopathological significance of BRAF-V600E, RET/PTC, TERT, RAS, and PAX8-PPAR-γ mutations in 300 patients with differentiated thyroid cancer (DTC). The patients were examined at the Tashkent City Branch of the Republican Specialized Scientific and Practical Medical Center of Oncology and Radiology between 2014 and 2024. All patients underwent cytology according to the Bethesda system, ultrasound stratification based on TI-RADS, and molecular analyses using PCR and FISH methods. The BRAF-V600E mutation was predominantly detected in papillary carcinoma (82.9%) and was associated with unfavorable TNM characteristics; RET/PTC occurred exclusively in PTC (100%); TERT was more frequent in the follicular type (46.2%); and RAS and PAX8-PPAR-γ mutations were observed only in FTC. The correlation with ultrasound features was weak. The findings confirm the importance of molecular genetic testing in DTC for improving prognostic accuracy and personalizing treatment strategies.
