CLINICAL EFFICACY OF MODIFIED AND ENDOVIDEOSURGICAL METHODS IN HERNIOPLASTY OF INGUINAL HERNIAS

The presented study substantiates the need for a personalized approach to the choice of surgical tactics for hernioplasty. Based on clinical, instrumental and functional parameters, a quantitative algorithm for risk stratification has been developed to optimize the choice between open and endovideosurgical methods. During the analysis of the treatment of 303 patients, it was shown that the introduction of a personalized algorithm for choosing a hernioplasty method made it possible to significantly reduce the incidence of early postoperative complications (from 19.8% to 6.2%) and relapses in the long term (from 9.8% to 1.7%). The use of a modified open tension-free hernioplasty using a hernial sac flap ensured a decrease in complications associated with damage to the structures of the spermatic cord and contributed to the preservation of reproductive function in men. Thus, the proposed personalized algorithm for choosing surgical tactics for inguinal hernias demonstrates high clinical effectiveness and can serve as a basis for an individualized approach in modern herniology.
