OPTIMIZATION OF FOCAL PERIODONTAL LESIONS TREATMENT WITH TOOTH-PRESERVING SURGERIES

Abstract

Objective — to evaluate the clinical efficacy of the modified Widman flap procedure combined with β-tricalcium phosphate application for the treatment of focal periodontal lesions compared to the standard flap surgery.

Materials and methods. A total of 74 patients (32 male, 42 female) aged 28–55 years with focal periodontal lesions were divided into two groups: main group (n = 38) — Widman flap surgery with β-TCP; comparison group (n = 36) — standard flap surgery. Probing depth (PD), clinical attachment level (CAL), bleeding on probing (BOP), oral hygiene index (OHI-S), and radiographic bone defect reduction were assessed at 3, 6, and 12 months.

Results. At 12 months, PD in the main group decreased from 6.4 ± 1.1 to 2.8 ± 0.7 mm (p < 0.001) versus 6.3 ± 1.0 → 3.7 ± 0.9 mm in the comparison group (p < 0.01); CAL gain was 3.2 ± 0.8 mm versus 1.9 ± 0.6 mm (p < 0.001); radiographic bone defect reduction reached 64.3 ± 8.7 % versus 38.2 ± 9.4 % (p < 0.001). Clinical stabilization was achieved in 92.1 % of patients in the main group versus 75.0 % in the comparison group (p < 0.05).

Conclusion. The application of β-tricalcium phosphate during flap surgery in patients with focal periodontal lesions significantly improves clinical and radiographic parameters and provides more stable long-term outcomes.
