MODERN APPROACHES TO THE TREATMENT OF COMPLICATED FORMS OF CHRONIC INFLAMMATION OF THE MIDDLE EAR

Abstract

Early diagnosis and effective treatment of patients with chronic inflammation of the middle ear and its complications remains relevant in the field of otolaryngology. Chronic purulent inflammation of the middle ear with complications is a serious disease that requires an integrated approach to diagnosis and treatment. This pathology includes various complications both locally and within the cranial cavity, which can seriously affect the prognosis and quality of life of patients.

We conducted a study whose purpose was to determine a strategy for comprehensive diagnosis and treatment of patients with chronic suppurative otitis media and its complications. Over the past 10 years, we have observed 114 patients with chronic suppurative otitis media, among whom 85 had local complications, and 29 had intracranial complications. All patients underwent a general clinical examination, computed tomography of the mastoid process, magnetic resonance imaging of the brain, as well as consultations with neurosurgeons, neuropathologists, ophthalmologists and other specialists.

All patients with purulent meningitis underwent a spinal puncture to clarify the diagnosis and administration of antibiotics. The patients' condition gradually improved, with clinical and biochemical parameters normalizing. This process was usually observed after surgical cleaning of the purulent focus of the mastoid process, drainage of brain abscesses, removal of blood clots from the sinuses and intensive drug therapy.

Due to the use of modern diagnostic methods and a comprehensive system of emergency surgical and intensive care, the prognosis of treatment results has improved and mortality has decreased in patients with chronic purulent otitis media with complications.
