MODERN ALGORITHMS FOR THE TREATMENT OF ACUTE AND CHRONIC CHOLECYSTITIS: FROM CONSERVATIVE THERAPY TO SURGERY

Abstract

A comparative analysis of laparoscopic and laparotomy cholecystectomy in patients with acute and chronic cholecystitis was performed in the study. Laparoscopic cholecystectomy was found to have several advantages: shorter duration of surgery (40.2 ± 7.5 min vs. 65.8 ± 11.7 min), shorter hospitalization (3.7 ± 1.2 days vs. 8.4 ± 2.1 days), and faster recovery (2-3 weeks vs. 4-6 weeks). The incidence of late complications, including postcholecystectomy syndrome, was lower in the laparoscopy group (5.1% vs. 12.8%), and the conversion rate was 5.1%. Laparotomy cholecystectomy remains an important method in complicated clinical cases such as severe inflammatory process and anatomical features of the biliary tract. These studies confirm high efficacy and safety of laparoscopic cholecystectomy, emphasizing the need to expand its use even in complicated forms of cholecystitis.
