The article presents a combined minimally invasive method for treating choledocholithiasis in patients with an atypical location of the major duodenal papilla (MDP). Based on a retrospective analysis of 132 cases, the high effectiveness of the method is demonstrated, which involves the antegrade insertion of a guidewire under ultrasound control. The results showed a reduction in the incidence of postoperative complications to 5.7% and an increase in the success rate of common bile duct (CBD) clearance to 97.1%. The method is recommended for patients with scarring deformations, diverticula, and tumor changes in the periampullary region.
