ASSESSMENT OF CIRCULATORY DISORDERS IN THE VERTEBROBASILAR ZONE IN KIMMERLE ANOMALY

Abstract

The data obtained from transcranial Doppler sonography (TCD) in 110 patients with radiologically detected Kimmerle anomaly (KA) were analyzed. Depending on the variants of the position and closure of the atlas bone ring, slowing down and asymmetry of blood flow velocity in the vertebral artery (VA) were determined. Using the normative indicators of blood flow velocity in the vertebral artery in patients with Kimmerle anomaly as a basis, various degrees of decreased blood flow velocity were identified. In cases of unilateral unclosed Kimmerle anomaly, hemodynamically significant changes in blood flow velocity were not observed. In cases of bilateral unclosed Kimmerle anomaly, the decrease in blood flow velocity was within the lower normative criteria.

The Kimmerle anomaly is clinically significant due to its association with the vertebral arteries and sympathetic nerve, as well as the potential manifestation of the vertebral artery syndrome. This is linked to potential vegetative-irritant perivascular sympathetic and ischemic mechanisms caused by reduced blood flow to the brain.

TCD examination during dynamic flow tests in V3 and V4 segments of the vertebral artery revealed a decrease in linear blood flow velocity, identical to that during ipsilateral lateral head rotation, especially in patients with bilateral complete Kimmerle anomaly.

In patients with a closed form of Kimmerle anomaly, hemodynamically significant changes in blood flow on that side were observed in all age groups, although it did not always lead to neurological symptoms.
