INNOVATIVE METHODS OF THERAPY FOR NEUROVASCULAR DISORDERS IN LUMBAR SPINE DORSOPATHIES

Purpose:The aim of the study is to diagnose neurovascular disorders in lumbar spine dorsopathies, their role in pathogenesis, and to improve treatment methods.
Materials and Methods:In the neurology department of the Samarkand Central Hospital, during 2022-2023, 180 patients were diagnosed with lumbar spine dorsopathy (110 in the main group, 70 in the comparison group), and 40 patients (control group) were examined without this condition. The study involved clinical-neurological examination, laboratory tests (homocysteine, IL10), neuroimaging (lumbar spine X-ray, MRI, CT), instrumental methods (Doppler ultrasound, EMG), pain intensity assessment scale (VAS), and statistical methods.
Results:The analysis of the conducted studies revealed certain signs in the development of vertebral syndrome, which predominated in patients with subjective dorsalgia. However, objective examination showed that in patients with lumbar dorsopathies and radicular syndrome, the pain score in the lower back was 2.8±0.3 points, while in the second group of patients, it was 2.6±0.2 points. In response to the intensity and duration of the pain syndrome, the first group showed predominance of myofixation indicators — the ipsilateral tension of the multifidus muscle symptom — 2.8±0.3 points, while in the second group, it was 2.5±0.2 points. The severity of myofixation in patients with lumbar dorsopathies, dorsalgias, and radicular syndromes was confirmed by determining the coefficient of vertebral syndrome. In the first group of patients, it was 11.5±1.1 units, and in the second group, it was 10.6±1.1 relative units, which corresponds to a severe degree.
Conclusion:The effectiveness of the treatment was analyzed based on the dynamics of neurological damage and neurophysiological study data. Repeated examinations of the patients showed positive dynamics in both groups, but the results were more pronounced in the group receiving additional treatment.
