BOLALARDA SURUNKALI PIELONEFRITNI KECHISH TENDENSIYASI VA OQIBATINI BASHORAT QILISHDA MOLEKULYAR TIBBIYOTNI ROLI

Tadqiqotning maqsadi: Surunkali piyelonefrit bilan kasallangan bemorlar guruhlarida siydik tarkibidagi KIM-1 ekskresiyasi darajasini aniqlash, shuningdek, kasallikni klinik kechish prognozi mezoni sifatida KIM-1 dan foydalanish imkoniyatini baholashdir.
Material va Metodlar: Tadqiqotga surunkali piyelonefrit bilan kasal bo’lgan 43 nafar, 7-17 yoshdagi bolalar olindi. Ilmiy tadqiqotni olib borishda qo‘yilgan vazifalarga qarab klinik, epidemiologik, biokimyoviy, statistik metodlar majmuasidan foydalanildi.
Natijalar: 1-guruh bemorlarda (n-21) turli darajada va lokalizatsiyada ifodalangan shish sindromi kuzatildi, proteinuriya kuniga bir martalik dozada 579,0 mg/kunni tashkil etdi, gipoalbuminemiya, eritrotsituriya kuzatildi. Barcha bemorlarda zardobdagi kreatinin darajasi o'rtacha 1,71±1,4 2 mg/dl tashkil etdi. 2-guruhda (n-22) kunlik protiyenuriya o’rtacha 282±4,85 mg/kun, eritrotsituriya Nechiporenko usuli bo'yicha 1 ml da 5000 dan oshmadi. KIM-1 darajasi 1-guruh bemorlarida 9,59 ng/ml (p<0,001) ni tashkil etdi. 2-guruh bemorlarida KIM-1 – 1,56 ng/ml bo’lib, bu statistik jihatdan ahamiyatsiz edi.
Xulosa. Siydikda KIM-1 miqdorini baholash buyrak parenximasining gipoksik, toksik va mikrobli zararlanishini erta aniqlashning arzon va noinvaziv usuli bo'lib, diagnostika sifatini oshiradi. KIM-1 biomarkerini siydikda aniqlashni yana bir afzalligi, bolada qon namunalarini olish bilan bog’liq asoratlarni, salbiy oqibatlarni, stressni kamaytiradi va bolaning hayot sifatini yaxshilashga yordam beradi.
