AORTOKORONAR SHUNTLASHDAN KEYINGI BEMORLARDA YALLIG'LANISHGA QARSHI TERAPIYANI TAKOMILLASHTIRISH

Sun'iy qon aylanishi (SQA) bilan yoki SQA siz o'tkazilgan kardioxirurgik operasiyalarda barcha bemorlarda yallig'lanish javob reaksiyasi rivojlanishi mumkin. Sun’iy qon aylanishi bilan bo’ladigan uzoq muddatli kardiojarrohlik amaliyoti davrida organizmda kapillyar o'tkazuvchanlikning oshishi rivojlanadi va bu esa organizmda keng tarqalgan shish, gaz almashinuvining buzilishi, miya, buyrak va jigar disfunktsiyalari rivojlanishiga olib keladi. Ushbu tadqiqotda bemorlar ulinastatinni qo'llanilishiga qarab ikki guruhga bo'lingan: 1-guruh (asosiy) n= 35 bemorlarida operasiya va operasiyadan keyingi erta davrlarda ulinastatin qo'llanilgan; 2-guruh (nazorat) n=30 intraoperativ va erta operasiyadan keyingi davrlarda ulinastatin qo'llanilmagan. Operatsiyadan keyingi yallig'lanishga qarshi terapiya majmuasida organlarning shikastlanishi va ko'p organlarning funksiyasining buzilishini oldini olish uchun keng spektrli proteaz ingibitori "ulinastatin" ni 500 ml 5% glyukoza yoki 0,9% natriy xlorid eritmasida 100000 XB dozada eritib tomir ichiga 1-2 soat davomida yuborish kerak. Ulinastatinni qo'llash fonida asosiy guruh bemorlarida proinflammator sitokinlar, xususan IL-6 sonining 8±1,6 PG/ml (p<0,05) ga kamayishi kuzatildi. Nazorat guruhida esa IL-6 ning 15±3,6 PG/ml (p>0,05) ga oshishi kuzatildi. Shunday qilib, o'tkazilgan tahlil shuni ko'rsatdiki ulinastatinning operasiyadan keyingi asoratlarning uchrash soniga yoki aortokoronar shuntlash operasiyasidan keyin intensiv terapiya bo'limida qolish muddatiga ijobiy ta'sirini ko'rsatadi.
