Abstract

Introduction. The article discusses the results of neuroprotective therapy in children with congenital glaucoma. The study demonstrated that the use of the drug "Cortexin" in combination therapy contributes to the stabilization of visual functions and reduces the risk of disease progression. Clinical and instrumental research methods confirming the effectiveness of the drug are presented.

The aim of the study was to evaluate the effectiveness of the neuroprotective effect of the drug "Cortexin" in congenital glaucoma.

Materials and Methods. The study included 44 children (88 eyes) with congenital glaucoma. Of these, 25 children (50 eyes) formed the main group that received the peptide bioregulator Cortexin. The remaining 19 children (38 eyes) constituted the control group.

Research methods included: visometry, ophthalmoscopy, biomicroscopy, perimetry, tonometry, and visual evoked potentials (VEP).

Results. To assess the dynamics of visual functions and stabilization of the glaucomatous process, psychophysical studies were conducted in all groups before the start of treatment, immediately after its completion, and six months later during the follow-up period. All patients had their intraocular pressure (IOP) reduced to a tolerable level after undergoing anti-glaucoma surgery. The study was prospective, with a follow-up period of 12 months. Patients who started therapy early showed more pronounced improvements in visual functions compared to those who began treatment at later stages of the disease.

Effectiveness at Different Disease Stages. The use of the neuroprotective drug Cortexin demonstrated varying effectiveness depending on the stage of congenital glaucoma. The greatest increase in visual acuity was recorded at the advanced stage, indicating an optimal therapeutic window for initiating neuroprotective treatment. This emphasizes the importance of early diagnosis and timely intervention. Perimetry showed improvement in the peripheral visual field in patients in the main group. No significant changes were observed in the control group. Additionally, improvements were noted in the stability of the central visual field and a reduction in absolute scotomas in the main group.

Conclusion. Thus, the psychophysical studies conducted showed that the use of Cortexin leads to the stabilization of visual functions in primary congenital glaucoma with compensated intraocular pressure at all stages of the disease. An increase in visual acuity, expansion of the visual field boundaries, and improvement in ophthalmoscopic parameters were confirmed.
