APPROACH TO TREATING CHILDREN WITH OTOMYCOSIS COMPLICATED BY CHRONIC SUPPURATIVE OTITIS MEDIA

Abstract

Otomycosis (fungal otitis externa) is usually characterized as a fungal infection of the external auditory canal, but in rare cases, it can also be complicated by the middle ear. Although it is not life-threatening, it is a complex and alarming disease for patients and otorhinolaryngologists, as it often requires long-term treatment and observation. Otomycosis is one of the most common diseases in general ENT practice, its prevalence among patients with symptoms of external otitis can reach from 9% to 27.2%, and in patients with ear discharge - up to 30%. This disease occurs throughout the world, but is particularly common in hot, humid, and dusty tropical and subtropical regions. The most common pathogens are Aspergillus niger and Candida albicans, but many other fungal species can also cause otomycosis. To diagnose, it is necessary to conduct a series of laboratory (microbiological, modern molecular, histological) and clinical studies, evaluate the function of the hearing organs, and identify the disease pathogen. In most cases, the treatment of otomycosis is limited to the use of local antifungal agents. They are prescribed based on laboratory test results. Treatment is conducted in 3 main stages: clearing the external auditory canal, etiological treatment against fungi and secondary infection, bacteria, and support measures for therapeutic procedures.
