This article examines the relationship between thyroid dysfunction and cognitive impairment, with a focus on hypothyroidism. The study involved 80 women with long-standing disease, divided into two subgroups: those with autoimmune thyroiditis and those in a post-total thyroidectomy state. Cognitive status was assessed using the Addenbrooke’s Cognitive Examination. The results showed that cognitive impairments were more frequent and pronounced in patients after thyroidectomy compared to those with autoimmune hypothyroidism. This is attributed to a more severe hormonal imbalance and reduced T3 levels in the brain due to levothyroxine monotherapy. The authors emphasize the importance of early detection of cognitive deficits and the need for individualized hormone replacement therapy to improve patients’ quality of life.
