CLINICAL AND NEUROIMAGING CHARACTERISTICS OF ACUTE CEREBRAL ISCHEMIA

Abstract

Purpose: To study the clinical and neuroimaging characteristics of acute cerebral ischemia.

Materials and Methods: patients were selected in a private hospital affiliated with Asaka Neurological Hospital Ltd. clinical neurological examination, neuroimaging study, cerebral vascular ultrasound, brain MRI (ANKE 1.5T) and MRA of brachycephalic arteries were performed, and patients were examined by NIHSS scale, Сollin index and Glasgow scale. A total of 74 patients were selected. All patients were divided into two groups. The first group of patients consisted of 40 patients with acute cerebral ischemia, their age was 46-80 years, mean age was 63±11,4 years, 22 of them were women, 18 were men. The second group consisted of 34 patients with atherothrombotic type of acute ischemic stroke, 12 women and 22 men.

Results: In the first group the diagnosis of acute cerebral ischemia was made in patients (n=40) according to such general cerebral symptoms as: headache character (headache of strong intensity (75%)), dizziness (90%), nausea (55%), vomiting (20%), insomnia (30%), neurological examination of patients did not reveal acute focal changes, there were no pathological reflexes. Rossolimo (100%) positive, M.Radovich, "Snout" and other reflexes of oral automatism (100%) positive, staggering in Romberg position (100%) was revealed.

The second group of patients consisted of patients with acute cerebral stroke of atherothrombotic type (n=34), the clinical picture of patients was characterized by such general cerebral symptoms as: headache (headache of severe intensity (35%)), dizziness (47%), nausea (12%), insomnia (20%), neurological examination revealed acute focal changes in patients: left-sided hemiparesis in 53% of patients, right-sided hemiparesis in 47% of patients, as well as pathological reflexes of Babinski (100%), Rossolimo (100%) positive, M. Radovich, "Snout" and other reflexes of oral automatism (100%) positive, staggering in the Romberg position was revealed (50%).

Conclusion: In patients with cerebrovascular disease, if the size of plaques in the vessels increases by 50%, it is recommended to consult a vascular surgeon or intervention surgeon, if this figure is less than 50%, recommend high-dose statins (depending on the liver enzymes such as ALT, AST in the blood), thus we can prevent the complications of cerebrovascular disease or stroke and reduce disability.
