This study was aimed at investigating the genetic, clinical, and laboratory parameters associated with the development of ischemic stroke in the context of COVID-19 infection. The study included 240 patients treated in clinical hospitals in Tashkent between 2020 and 2023. In all cases, SARS-CoV-2 infection was confirmed by PCR or antigen testing. The patients were divided into two groups: the main group with ischemic stroke (n = 120) and the control group without neurological complications (n = 120). During the study, polymorphisms of the MTHFR C677T, TNF-α rs1800629, IL-6 rs1800795, IL-1β rs1143627, and VEGF rs2010963 genes, levels of cytokines (IL-6, TNF-α, IL-1β, VEGFA), and clinical indicators (disease severity, neurological deficit, infarct volume) were examined. The obtained results confirmed that inflammation and thrombogenic mechanisms, driven by genetic factors, play an essential role in the pathogenesis of ischemic stroke associated with COVID-19.
