PARKINSONIZM SINDROMINING NOQULAY KECHISHI: ZAMONAVIY PREDIKTORLAR VA PERSONALLASHTIRILGAN PROFILAKTIKA IMKONIYATLARI

##article.abstract##

Parkinsonizm sindromi klinik va patogenetik jihatdan geterogen holatlar guruhi bo‘lib, unda noqulay prognoz nafaqat bradikineziya, rigidlik va tremorning yaqqolligi bilan, balki kognitiv-regulyator, vegetativ, postural, neyrosomatik hamda ijtimoiy-funksional buzilishlarning to‘planib borishi bilan ham belgilanadi. Kasallikning progressivlashuviga oid zamonaviy konsepsiya alohida simptomlarni baholashdan kasallikning individual trayektoriyasi va klinik ahamiyatga ega bosqichlarini tahlil qilishga o‘tmoqda. Funksional-latent bosqich, funksional-manifest kognitiv-regulyator dezadaptatsiya va a’zolararo aralash dekompensatsiyani ajratib ko‘rsatish alohida ahamiyat kasb etadi. Ushbu sharhda kasallikning progressivlashuviga oid klinik, neyropsixologik, neyrofiziologik, neyrovizualizatsion va biologik markerlar, jumladan, NfL, GFAP, fosforlangan tau-oqsil, Aβ42/40, neyrotrofik va immun-yallig‘lanish ko‘rsatkichlari, shuningdek, fosforlangan α-sinuklein haqidagi zamonaviy ma’lumotlar tizimlashtirilgan. Funksional zaxiraning kamayish komponentlari sifatida yurishning raqamli parametrlari, vegetativ disfunksiya, suyaklarning mo‘rtlashishi va nutritiv buzilishlar ko‘rib chiqilgan. A’zolararo aralash dekompensatsiya bosqichiga o‘tishning oldini olishga qaratilgan ko‘p bosqichli xavf stratifikatsiyasi va personallashtirilgan profilaktikaning maqsadga muvofiqligi asoslab berilgan. Taklif etilayotgan yondashuv amaliy sog‘liqni saqlashda qo‘llash uchun qulay bo‘lgan hamda orqa miya suyuqligini majburiy tekshirishni talab etmaydigan integral prognostik modelni yaratish uchun asos bo‘lib xizmat qilishi mumkin.
